Job summary

Compassus is seeking a Hospice Clinical Liaison to promote our values and support referral sources in identifying patients needing hospice care. The role emphasizes coordination of admissions, patient education, and collaboration with hospitals and facilities to ensure smooth transitions.

The ideal candidate is a registered nurse with 2–3 years of related experience, strong communication skills, and a passion for compassionate care.

Qualifications

• RN license required.
• 2–3 years of related experience.
• Hospital and/or long-term care clinical experience preferred.
• Experience in healthcare marketing preferred.

Responsibilities

• Attend staff meetings and community events to represent Compassus.
• Explain hospice services and benefits to patients and families.
• Coordinate admissions and ensure all forms are completed and filed.
• Develop relationships with hospitals, facilities, and referral sources.
• Coordinate patient discharge from hospital to home and arrange needed services.
