Job summary

Affinity Health Management is seeking a Hospice Chaplain to provide spiritual care to patients and families, either directly or through coordination with other spiritual counselors.

You will assess spiritual needs, develop individualized care plans, and work with an interdisciplinary team to ensure compassionate care. Interfaith collaboration, strong communication, and community outreach are essential for this role.

Qualifications

• Bachelor's degree in religious studies or related field and a master's in theology, divinity, or pastoral studies.
• Certification preferred; ordained or commissioned to minister and must present endorsement letter for chaplaincy.
• Ongoing commitment toward education and obtaining CPE.
• Hospice experience preferred; comfort in an interfaith setting.

Responsibilities

• Assesses spiritual needs of patients and families; ensures timely services.
• Participates in comprehensive assessment to tailor the plan of care.
• Provides direct spiritual care to patients and families/caregivers.
• Liaises with community chaplains and spiritual counselors for coherence of care.
• Maintains records of spiritual care activities for QA and policy review.
• Documents services and communicates with community counselors; attends IDG meetings.
• Educates and supports the interdisciplinary group on spiritual care.
• Recruits community chaplains and volunteers to meet patient needs.
