Job summary

Residential Home Health and Residential Hospice is hiring a Hospice Certified Nursing Assistant (CNA) to provide compassionate in-home care for patients in the Mount Carmel, IL area. You will work with an interdisciplinary team to support comfort, dignity, and quality of life.

The role requires an active CNA certificate in the state, a high school diploma, at least one year of healthcare experience, and a valid driver's license for travel to patient homes.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Driver’s license and ability to drive ~20% of workday to patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team.
• Provide hands-on care to patients, assisting with ADLs and observing changes in condition.
• Support and guide patients and caregivers, reinforce the care plan and explain hospice journey.
• Respond promptly to patient needs, document care in the EMR accurately.
• Work independently, but collaborate with team and travel to patient homes within territory

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
