Residential Home Health and Hospice is seeking a Certified Nursing Assistant to provide hospice care in patients' homes in the Belleville area. You will deliver compassionate, patient-centered care and work with an interdisciplinary team to ensure comfort and quality of life.

The role requires a valid CNA certificate, a high school diploma, and at least one year of healthcare experience with willingness to drive to patient locations about 20% of the day.

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Valid driver’s license and ability to drive to patient locations ~20% of workday

Responsabilités

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory

CNA Qualifications
