Job summary

Residential Home Health and Residential Hospice is seeking a Hospice Certified Nursing Assistant (CNA) to deliver compassionate, patient-centered care in a home-based hospice setting in Illinois. You will collaborate with an interdisciplinary team to support comfort, dignity, and quality of life for patients and families.

You will provide hands-on care, assist with ADLs, observe and report changes, and document care in the EMR while traveling to patient homes within your territory.

CNA Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
