Job summary

Residential Home Health and Hospice in Mount Carmel, IL seeks a Certified Nursing Assistant to provide compassionate hospice care in patients' homes. You will support ADLs, monitor condition, and document in the EMR while traveling within your territory.

You will work independently with a supportive team, earn comprehensive benefits, and have opportunities to advance, including Purdue degree partnerships.

Qualifications

• Active CNA certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience, Hospice experience preferred.
• A valid driver’s license and ability to travel ~20% of the workday.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
