Job summary

Residential Home Health and Hospice in Springfield, IL is seeking a dedicated Hospice Certified Nursing Assistant (CNA) to provide patient-centered care in home settings. You will work with an interdisciplinary team to support comfort and quality of life, perform ADLs, and document care in the EMR.

The role requires an active CNA in the state, at least one year of healthcare experience, and a valid driver’s license for travel to patient homes within the assigned territory.

Qualifications

Responsibilities

CNA Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
