Job summary

Residential Home Health and Hospice is hiring a Certified Nursing Assistant for a Hospice position serving patients in their homes around Elgin, IL. Eligible for a $5,000 Sign-on Bonus.

Qualifications

• Active Certified Nursing Assistant (CNA) certificate in the state you work.
• High School Diploma or certificate.
• Minimum one year healthcare experience; Hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice home setting.
• Provide hands-on care to patients and assist with ADLs.
• Reinforce the care plan and offer emotional support to patients and caregivers.
• Document care provided accurately in the EMR.
• Work independently, but stay connected with the team.
• Travel to patient homes within assigned territory.

What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life

• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey

• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR

• Work independently, but never alone

• Travel to patient homes within assigned territory
