Job summary

Residential Home Health and Hospice in Naperville, IL is hiring a full-time Certified Nursing Assistant to deliver compassionate in-home hospice care. What You’ll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands‑on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
