Job summary

Advocate Health in Wisconsin seeks a Registered Nurse to coordinate compassionate hospice care and support for patients and families. You will lead the interdisciplinary group, develop and update care plans, and ensure documentation meets all regulatory standards.

The role requires nursing licensure, 1–2 years in related settings, and the ability to educate families while coordinating services and ensuring safe, effective care for diverse patient needs.

Qualifications

• Completion of an accredited nursing program.
• 1-2 years of nursing experience in home health, hospice, medical/surgical, LTC, ER or oncology.

Responsibilities

• Identify and coordinate patient/family care to support terminally ill patients and families.
• Educate patients and families regarding disease processes, hospice philosophies, and end-of-life care.
• Lead the IDG and develop the plan of care for each patient on caseload.
• Document in the electronic health record per agency standards and guidelines.
• Supervise Home Health Aide and LPN supervisory visits as required.
• Facilitate discharge planning and coordinate services after discharge.
• May conduct OASIS assessments for Palliative Care patients in WI.
