Job summary

Qualifications

• Registered Nurse license in the practicing state.
• Completion of an accredited nursing program.
• BLS for Healthcare Providers certification or equivalent on hire.

Responsibilities

• Performs all scheduled visits and collaborates with the interdisciplinary team.
• Conducts comprehensive physical, psychosocial and environmental assessments and updates the care plan.
• Communicates with physicians to update plans and informs the RN Case Manager of changes.
• Assesses patient response to treatments and revises the care plan as needed.
• Partners with the RN Case Manager for discharge planning and uses community resources for continuity of care.
• Documents and updates patient records per agency, state and federal guidelines.
• Communicates problems to physicians and RN Case Manager and states plans and goals clearly.
• Follows policies and monitors changes; performs patient positioning and transporting within safety limits.
• Demonstrates knowledge of age-specific care throughout the patient lifespan.
