Affinity Health Management is seeking a Registered Nurse for hospice care, directing services to meet home and community needs. You will assess patients and families, develop and adjust plans of care, and coordinate with physicians and the interdisciplinary team to ensure quality, compassionate care at home.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice in Ohio.
• One year of professional nursing experience within the last three years, preferably in home health, hospice, or public health.

Responsibilities

• Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process and revise the plan of care as needed.
• Write and initiate care plans; regularly re-evaluate patient and family needs and coordinate with physicians.
• Initiate plan of care revisions as patient status changes and consult with team.
• Provide health care instructions and assist patient with daily living activities while promoting self-sufficiency.
