Affinity Health Management is seeking a Hospice Registered Nurse Case Manager to plan, organize, and direct hospice care within homes and communities. You will manage comprehensive patient assessments, develop care plans, administer prescribed treatments, and coordinate with the hospice team to ensure quality care and family support.

The role emphasizes community health education, ongoing evaluation, and on-call responsibilities to meet patient needs in a compassionate, professional manner.

Formación

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing.
• Experience in public health, home care, or hospice nursing preferred.

Responsabilidades

• Conduct initial, ongoing patient and family assessments to determine hospice needs.
• Develop and revise care plans based on nursing diagnoses and patient/family input.
• Provide professional nursing care and administer medications as prescribed.
• Coordinate patient care with hospice interdisciplinary team and supervise aides.
• Participate in on-call duties and ensure safe, effective care delivery.
