Job summary

PeaceHealth seeks a Clinical Supervisor for Home Care Services – Hospice Medicare Certified, full-time, day shift. The role provides clinical oversight for the multidisciplinary team delivering home health, hospice, and pallative care services, ensuring quality patient outcomes and compliance with CMS and state guidelines.

Responsibilities include supervising direct care staff, coordinating care across teams, contributing to budget planning, and preparing reports to inform program evaluations.

Qualifications

• Nursing degree or equivalent required.
• Physical therapy, occupational therapy, speech-language pathology, or social work degree per Accredited School requirement.
• Licensure in WA state where applicable.

Responsibilities

• Supervise and coordinate daily operations of the clinical services team.
• Lead, coach and train direct patient care staff; participate in performance evaluations.
• Coordinate care delivery to ensure quality patient outcomes and regulatory compliance.
• Ensure documentation aligns with care plans and regulatory standards.
• Manage budgets, productivity, and communications across care teams.
