Qualifications

• Graduate of an approved school of clinical education.
• Licensed to practice in the state.
• At least one year of home health experience.
• Knowledge of federal, state and local hospice laws and regulations.
• Proficiency with mobile devices and basic technology.
• Experience or knowledge of hospice is preferred.

Responsibilities

• Represent the branch in external transitional care activities to promote Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans using clinical guidelines and metrics.
• Educate external groups about provided services.
