Job summary

St. Croix Hospice is seeking a Care Transition Coordinator in Livonia, MI to build relationships with referral sources and communities while educating on hospice care. The role supports intake to ensure timely referral coordination and contributes to marketing plan development.

You will coordinate daily sales and marketing operations, monitor outcomes, and help educate patients and families. This position emphasizes collaboration with the Director and care teams in a fast-paced environment.

Qualifications

• Associate degree in Marketing, Business Administration, Healthcare or related field.
• Ability to position services to customers and deal tactfully with clients.
• Demonstrates good communications, negotiation and public relations skills.
• Autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Ability to pass DHS background study.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider of services.
• Document all sales activity in the CRM per organizational requirements.
• Employ marketing and promotional initiatives to achieve budgetary sales volume projections.
• Assists with coordinating and scheduling marketing events, vendor fairs, and exhibits within various territories.
• Assist with ongoing training and support of Care Transition Team and orient new staff.
