Job summary

St. Croix Hospice in Bloomington, MN is seeking a Care Transition Coordinator to develop strong relationships with referral sources and designated communities, while educating clients on hospice care. You will assist intake to ensure timely referral coordination.

You will meet admission targets, coordinate daily sales and marketing operations, and maintain CRM records. This role supports budgeting, market assessments, and collaboration with the care team to improve patient access and outcomes.

Qualifications

• Associate's degree required in Marketing/Business/Healthcare or related field.
• Ability to position services to customers and clients.
• Strong communication, negotiation, and public relations skills.
• Autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider of services.
• Document all sales activity in the CRM per organizational requirements.
• Employ marketing and promotional initiatives to achieve budgetary sales volume projections.
• Maintain knowledge of markets, government agencies, payer groups, referral sources, and competitors.
• Enhance communication among patient, family, caregiver, facility, physician team, and hospice vendors.
• Educate patients and families on hospice to support informed decisions.
