Job summary

VitalCaring Group is seeking a Care Transition Coordinator – Hospice to connect patients, families, and healthcare partners and to drive seamless transitions into home health or hospice services.

The role blends healthcare sales, care coordination, and partnership development in a field-based setting with travel across the South Dallas territory. A strong background in sales and relationship-building is expected.

Qualifications

• Bachelor’s degree preferred (or equivalent experience)
• 2+ years healthcare or related sales experience
• Strong communication and influencing skills
• Proficient in MS Office and CRM
• Valid driver’s license and reliable transportation

Responsibilities

• Liaise between referral sources, patients, and clinical teams to ensure seamless transitions
• Conduct structured sales calls with planning and relationship-building
• Build partnerships with hospitals and referral sources
• Develop strategies to grow market share and referrals
• Document sales activity in CRM/EMR
