Job summary

Enhabit Home Health & Hospice is seeking a compassionate clinician to coordinate patient transitions and connect with families, physicians and care facilities. You will promote available home health services and uphold quality standards while navigating regulatory guidelines.

We value licensed clinicians with at least one year in home care and comfort with mobile technology to support documentation and patient care coordination.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year of experience in home care.
• Knowledge of applicable federal, state, and local laws and regulatory guidelines for hospice operations.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and in professional contacts with patients, families, physicians, hospitals, facilities and other health groups to promote Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines, protocols and metrics to improve quality and efficiency.
