Job summary

St. Croix Hospice seeks a Care Transition Coordinator to strengthen referrals and oversee education for patients, families, and caregivers. You will coordinate admissions, CRM activity, and marketing events to support budgets and growth.

You will build relationships with referral sources and communities while guiding teams in care transition activities and ensuring timely, accurate coordination of referrals.

Qualifications

• Associate degrees in Marketing, Business Admin or Healthcare.
• Ability to position services to customers with tact.
• Good communications, negotiation and public relations skills.
• Autonomy, organization, assertiveness and flexibility.
• Ability to pass DHS background study.
• Physical requirements apply (pushing/pulling, lifting up to 10 lbs).

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations and marketing initiatives.
• Build and monitor perceptions of St. Croix Hospice as a high-quality provider.
• Document all sales activity in the CRM per requirements.
• Assist in budgeting and resource allocation for marketing.
• Develop market knowledge of referral sources and competitors.
• Provide education to patients, families, and caregivers about hospice.
• Assist in training Care Transition Team and orient new staff.
• Coordinate and schedule marketing events and vendor fairs.
• Support territory management and strategic planning.
