Heart of Hospice in Fort Smith, part of the LHC Group and Optum family, is seeking a Care Coordinator for hospice services. The role focuses on scheduling, coordinating patient visits, and ensuring care continuity under IDG guidelines.

Qualifications

• 1+ years of scheduling experience in health care setting using an online scheduling system.
• Demonstrated exceptional organizational, customer service, communication, and decision-making skills.
• Working knowledge of state and federal regulations governing hospice IDG requirements and supervisory visit requirements.

Responsibilities

• Helps manage order tracking, IDG preparation, and other tasks associated with patient care management.
• Monitors pending referrals daily and assigns RN or APRN for start of care visits.
• Monitors productivity and distribution of visits and communicates changes with the clinical leader.
