Job summary

Arkansas Hospice, Inc. in North Little Rock, AR seeks a Clinical Coordinator to coordinate the interdisciplinary Plan of Care for patients and families, addressing physical, psychosocial, spiritual, and economic needs.

The role partners with the care team to deliver high-quality services and outstanding customer support. The position requires an active background in direct patient care (CNA/LPN/MA) and strong communication.

Qualifications

• Education: High school diploma or GED; completion of CNA, LPN, or MA program.
• Experience: Direct patient care in hospital, long-term care, physician office, or hospice preferred.
• Licensure: CNA, LPN, or MA certification/licensure.

Responsibilities

• Provides safe, high quality coordination of care and plans of care for patients and families.
• Performs ongoing physical and emotional assessment and updates the plan of care as needed.
• Coordinates services (DME, home health, hospice) throughout transitions of care.
• Communicates with patients, families and team members to ensure coordinated care.
• Maintains accurate patient records and documentation of visits and actions.
• Acts as liaison between partner facilities and Arkansas Hospice and its care teams.
• Contributes to team meetings, case conferences, and care plan discussions.
