Job summary

UnitedHealth Group's Northern Arizona Hospice is seeking a Care Coordinator to help manage patient care scheduling and coordination in the Flagstaff area. You will work within the LHC Group network to support hospitals, physicians and families, ensuring timely care and smooth transitions for patients at home.

Ideal candidate has 1+ year scheduling in home health or hospice, strong organizational and communication skills, and knowledge of hospice regulations.

Qualifications

• 1+ year of scheduling experience in home health or hospice care using an online system.
• Strong organizational, customer service, communication, and decision-making skills.
• Working knowledge of state and federal hospice regulations (IDG, supervisory visits).

Responsibilities

• Manage order tracking, IDG prep and other patient care management tasks.
• Monitor referrals daily and assign RN or APRN for start of care visits.
• Track productivity and adjust visit distribution with the clinical leader.
• Coordinate patient visit schedules with the RN Case Manager and Social Worker to ensure continuity.
• Assist with scheduling and tracking visits to meet hospice policies and regulatory requirements.
