Job summary

Trustbridge Hospice, part of Empath Health, is seeking a Care Navigation Manager to lead the intake process for hospice referrals in West Palm Beach. This on-site role supports admissions, coordinates initial visits, and ensures compassionate, compliant patient experiences during a pivotal time.

The ideal candidate has 3+ years in hospice/admissions, supervisory experience, and strong communication. A bachelor’s degree in a related field is preferred.

Qualifications

• Bachelor’s degree preferred in nursing, healthcare administration, social work, or related field.
• Minimum 3 years of hospice, home health, or healthcare intake/admissions experience.
• 1–2 years of supervisory experience in healthcare.
• Exceptional communication and interpersonal skills with a compassionate, patient-centered approach.
• Familiarity with hospice philosophy, eligibility criteria, and regulatory guidelines.
• Proficiency in EHR systems and Microsoft Office Suite.

Responsibilities

• Oversee the care navigation team responsible for receiving and processing all hospice referrals.
• Ensure prompt and accurate intake of patient information, including clinical details and insurance verification.
• Provide clear explanations of hospice services, goals, and eligibility.
• Schedule initial visits and admissions assessments in collaboration with the admissions team.
• Monitor referral-to-admission timelines to ensure timely access to care.
• Maintain accurate records in the EHR and ensure HIPAA compliance.
• Train and supervise intake staff to uphold high standards of service.
• Identify barriers to timely admission and resolve access issues.
• Develop standard operating procedures for intake with a focus on quality improvement.
• Generate reports on referral volume and key performance indicators.
