Job summary

Trustbridge Hospice, part of Empath Health, seeks an Admissions Care Partner in West Palm Beach. This on-site, full-time role supports patients and families through the admissions process and continuum of care, from intake to field coordination, ensuring a smooth transition during a critical stage of care.

You will present the hospice program, process referrals, obtain consents, and respond to patient needs, upholding Empath Health values and a mission of Full Life Care for All.

Qualifications

• Bachelor's degree preferred in a health-related or human services field.
• Experience in customer service/referral coordination in healthcare or community settings is desirable.
• Strong understanding of medical terminology preferred.
• Hospice experience highly desirable.

Responsibilities

• Present our hospice program and continuum of care to families/patients and facilitate timely admissions.
• Process referrals, contact pending list calls, obtain consents, and complete non-nursing documentation.
• Assist with admissions from Intake to the Field and respond to patient needs while upholding Trustbridge values.
• Act as a problem solver to provide continuity of care for patients and families.
• Coordinate the plan of care and treatment for hospice-appropriate patients and families.
• Articulate the organizationontinuum of care to guide patients and families to services.
