Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate Registered Nurse on the Admissions Team to support Broward County patients and families. You will evaluate referrals, educate families about hospice care, and coordinate care plans with physicians and healthcare partners.

Qualifications

• Florida RN license is required.
• BSN preferred for applicant.
• Hospice certification preferred.
• Current Basic Life Support (BLS) certification required.
• Experience in public health, home health, oncology, med/surg, geriatric, or hospice nursing preferred.
• Strong assessment, communication, and teaching skills; proficient with EMR.
• Reliable transportation with valid Florida driver's license.

Responsibilities

• Evaluate patients referred for hospice services.
• Educate patients and families on hospice care and continuum of care.
• Coordinate individualized plans of care with referring physicians and liaisons.
• Serve as liaison between Trustbridge Hospice and hospitals, physicians, and long‑term care facilities.
• Promote hospice philosophy through education and relationship-building.
• Ensure timely, compassionate care and support for families throughout hospice journey.
