Trustbridge Hospice, a member of Empath Health, is seeking a compassionate Registered Nurse to join our Admissions Team serving Palm Beach County. This full-time role requires Florida RN licensure, BSN preferred, and hospice certification preferred.

Qualificações

• Must be a licensed Registered Nurse in Florida.
• BSN preferred and hospice certification preferred.
• Current Basic Life Support (BLS) certification required.

Responsabilidades

• Evaluate patients referred for hospice services.
• Educate patients and families on hospice care and the continuum of care.
• Collaborate with referring physicians and liaisons to coordinate individualized plans of care.
• Act as a liaison between Trustbridge Hospice and hospitals, physicians, long‑term care facilities, and other healthcare professionals.
• Promote the hospice philosophy through education and relationship-building within the healthcare community.
