Job summary

Heartland Hospice is seeking an experienced Hospice Clinical Liaison to coordinate intake, admissions, and referrals with hospitals, physicians, and discharge planners. You will educate patients and families about hospice care, review medical records, and ensure compliant, timely processes.

You will build strong relationships with referral sources and collaborate with the hospice team to support seamless patient transitions and advocacy for quality care.

Qualifications

• Licensed Nurse with 3–5 years of hospice experience required.
• BSN with 5+ years hospice experience preferred.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other healthcare professionals.
• Assess patient eligibility for hospice services by reviewing records and communicating with physicians and families.
• Educate patients and families on hospice care, processes, and benefits, with clarity and compassion.
• Complete documentation and coordinate intake/admission, ensuring legal, clinical, and payer requirements are met.
• Serve as liaison between referral sources and the hospice care team, ensuring seamless handoffs and rapid response.
• Make daily visits to hospitals and facilities to maintain visibility with discharge planners and physicians.
• Build and maintain relationships with referral partners and identify new outreach opportunities.
