Job summary

Heart to Heart Hospice in Texas is seeking a Customer Service Liaison to promote a patient- and family-centered hospice presence across healthcare facilities. You will coordinate care among clinicians, communicate with patients and families, and ensure high satisfaction with our services.

This full-time role requires 2–3 years of healthcare experience, a valid driver’s license, and reliable transportation. EMR experience is a plus; you will collaborate with leadership to meet growth targets and

Qualifications

2 - 3 years of healthcare experience.
Experience in hospice, home health, nursing homes, assisted living, or hospital preferred.
Current State Driver’s License and reliable transportation.

Responsibilities

Promote a customer-focused hospice presence in the healthcare community across facilities and hospitals.
Coordinate care among healthcare professionals.
Communicate with patients and families regularly to ensure needs are met and high satisfaction.
Identify problems or dissatisfaction and ensure quick resolution with clinical team.
Coordinate in-services or educational events for families or facilities.
Work with agency leadership to meet growth and development targets.
Flexible scheduling for after-hours and weekend consultations.
Ensure compliance with local, state, and federal laws and Medicare regulations.
