Job summary

St. Croix Hospice in Lombard, IL seeks a Care Transition Coordinator to develop and maintain relationships with referral sources and educate stakeholders on hospice care.

You will support intake to ensure timely referrals and collaborate with the care team to align services with patient needs. Responsibilities include meeting admission targets, coordinating marketing operations, updating CRM, presenting education to families, assisting budget planning, and coordinating events.

Qualifications

• Ability to pass DHS background study.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily marketing operations including initiatives.
• Document all sales activity in the CRM.
• Educate patients, families, and caregivers about hospice and informed decision making.
• Assist Director in budgeting and marketing allocations.
• Monitor market and budget allocations and plan marketing activities.
• Coordinate and schedule marketing events, vendor fairs, and exhibits.
• Mentor and shadow Care Transition Coordinators to assist in territory management.
