Job summary

VITAS Healthcare in Florida is seeking a Registered Nurse, RN, to coordinate care for hospice patients and families, performing comprehensive assessments, and implementing the interdisciplinary plan of care to ensure optimal comfort and quality of life.

The RN will provide education to patients and families, collaborate with IDG members, and participate in on-call rotations and team meetings to support compassionate end‑of‑life care within the hospice program.

Qualifications

• Licensed to practice as a Registered Nurse in the state where the VITAS program operates.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.

Responsibilities

• Case management and supervision for RN within the hospice team.
• Follow VITAS policies and standards and practice within the scope of the RN license.
• Complete initial and ongoing comprehensive assessments for palliation and care planning.
• Develop, coordinate, and follow the patient's individualized plan of care with IDG.
• Provide education to patients, families and caregivers on pain management and care needs.
• Participate in on-call rotation and after-hours shifts as needed.
