Job summary

VITAS Healthcare is seeking a Registered Nurse (RN) to join our hospice interdisciplinary team in Florida. The RN will coordinate care, perform ongoing patient assessments, and implement the interdisciplinary plan of care to support terminally ill patients and families.

The role requires an active RN license in the state, at least two years of hospice/home health experience, and the ability to supervise nursing tasks delegated to aides and other staff.

Qualifications

• Current RN license in the state where the VITAS program is located.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered.

Responsibilities

• Case management and supervision for Registered Nurse, RN.
• Follows VITAS policies and standards and practices within the scope of the RN license.
• Performs supervision of nursing tasks delegated to Hospice Aides, LPNs, and Volunteers.
• Completes initial and ongoing comprehensive assessments across physical, psychosocial, spiritual, and bereavement areas.
• Identifies resources and referrals (PT, OT, Home Health, Dietary, Pharmacy, equipment).
• Completes documentation at time of event and at the location of care.
• Provides education and training to patients and families, including pain management.
• Participates in patient care meetings and weekly team meetings.
• Maintains effective communication and shift reports.
• Participates in bereavement support and on-call coverage as needed.
• Works with IDG to develop and follow the patient’s plan of care.
• Obtains physician orders when necessary and delivers services requiring nursing skill.
