Job summary

Qualifications

• Current RN license in the state of practice.
• Minimum two years of nursing experience in hospice, home health, or community health in the last five years.

Responsibilities

• Case management and supervision for RN
• Performs comprehensive assessments including physical, psychosocial, spiritual, and bereavement care
• Coordinates and follows the individualized plan of care with the IDG
• Provides patient and family education on pain management and symptom control
• Participates in QAPI and quality improvement activities
• On-call rotation and after-hours coverage as needed
