Job summary

VITAS Healthcare in Fort Myers, FL seeks an experienced Registered Nurse (RN) to coordinate hospice care within an interdisciplinary team. You will assess patient and family needs, implement the plan of care, and supervise nursing tasks delegated to aides and licensed staff.

The role requires an active RN license in the state and at least two years in hospice, home health, or community health within five years. Travel to coverage areas and on-call participation may be required.

Qualifications

• Currently licensed to practice nursing in the state where the VITAS program is located.
• A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered

Responsibilities

• Case Management and Supervision for Registered Nurse, RN
• Follows VITAS policies and standards and practices within the scope of the RN license
• Reports any request to practice outside of scope of license to the manager/supervisor
• Performs supervision of nursing tasks delegated to Hospice Aides, Licensed Practical Nurses and Volunteers
• Completes initial and ongoing comprehensive assessments to provide palliation of end-of-life symptoms
• Identifies resources and referrals such as PT, OT, Home Health, etc.
• Completes documentation at the time of the event and at the location of care
• Provides education and training to patients, families, caregivers and referral sources
• Participates in patient care meetings and weekly team meetings
• Provides effective communication and shift reports
• Participates in bereavement support and on-call rotation
• Provides visits/shifts in program coverage area
• Works with IDG to develop and follow the patient’s plan of care
• Obtains physician orders when necessary
• Provides services requiring substantial and specialized nursing skill
• Evaluates outcomes of services and delegates tasks appropriately
• Participates in Quality Improvement activities and Professional Development
• Represents the nursing profession and VITAS positively
• Maintains focus on patient and family experience of care
• Performs duties in alignment with patient safety and care standards
