Job summary

VITAS Healthcare in Kissimmee, FL is seeking a Registered Nurse (RN) to coordinate care for hospice patients and support families facing terminal illness.

You will perform comprehensive assessments, implement the interdisciplinary plan of care, educate patients and families, and participate in team meetings.

A current RN license and at least two years in hospice or home health are required; reliable transportation is expected, with possible on-call shifts as needed.

Qualifications

• Current nursing license in the state of practice.
• Minimum two years of nursing experience in hospice, home health, or community health in the last five years.
• Reliable transportation with adequate insurance coverage.

Responsibilities

• Case Management and supervision for RN.
• Follow VITAS policies and standards and practice within the scope of the RN license.
• Complete initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement.
• Coordinate, follow up, and implement the interdisciplinary plan of care.
• Provide education and training to patients, families, caregivers and referral sources.
• Participate in patient care meetings and weekly team meetings.
• On-call rotation and shifts during non-business hours as needed.
