Job summary

VITAS Healthcare in Miami, FL is seeking a Registered Nurse, RN to join our interdisciplinary team and coordinate comprehensive hospice care for patients and families facing terminal illness and bereavement.

The role requires an active nursing license in the state, at least two years of hospice/home health experience in the last five years, and the ability to work evenings, weekends, or holidays as needed. You will assess needs, collaborate with the IDG, and support care planning and education.

Qualifications

• Active nursing license in the state where the VITAS program is located.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Equivalent experience or licensure may be considered

Responsibilities

• Case Management and Supervision for Registered Nurse, RN
• Follows VITAS policies and standards and practices within the scope of the RN license
• Reports any request to practice outside of scope of license to the manager/supervisor
• Performs supervision of nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses and Volunteers
• Completes initial and ongoing comprehensive assessments to provide palliation of end-of-life symptoms
• Identifies/utilizes resources and referrals
• Completes documentation at the time of the event and at the location of care
• Provides education and training to patients, families, caregivers and referral sources
• Participates in patient care meetings and weekly team meetings
• Provides shift reports and on-call coverage as needed
• Participates in bereavement support
• Visits/shifts in program coverage area
• Works with IDG to develop, coordinate, and follow the plan of care
• Obtains physician orders when necessary
• Delivers nursing services with substantial and specialized nursing skill
• Evaluates outcomes and delegates tasks as appropriate
• Participates in Quality Improvement activities and professional development
