Job summary

Compassus is seeking a Hospice Team Coordinator to oversee patient data, records, and interdepartmental coordination. You will ensure accurate admission documentation, upload records into HCHB/EMR, and support the interdisciplinary team to deliver high-quality hospice care.

The role requires a high school diploma or GED and 2+ years in medical records or healthcare office support, with strong communication and organizational skills.

Qualifications

• Must have strong data entry and record-keeping skills.
• Excellent written and verbal communication abilities.
• Experience with medical records and healthcare office support preferred.

Responsibilities

• Accurately enters and manages patient data including referral and demographics.
• Ensures admission docs are complete with CTI and orders to admit.
• Uploads documents to HCHB and maintains patient records in EMR.
• Generates regulatory reports and adds coordination notes as needed.
• Monitors DME orders and informs management of trends or issues.
• Coordinates with IDT and acts as central contact for team, patients, and families.
