Compassus is seeking a Hospice Team Coordinator to oversee delivery of hospice care, maintain patient records, and coordinate with the interdisciplinary team to ensure quality, compassionate care. You will manage intake, admissions, and billing-related documentation, while serving as a primary contact for patients and families.

Qualifications

• High school diploma or GED required.
• Experience with medical records or medical office support preferred.
• Strong written and verbal communication skills.
• Ability to maintain confidential information and coordinate with teams.

Responsibilities

• Accurately enters and manages patient data in the EMR/HCHB systems.
• Ensures admission documents are complete with physician CTI and order to admit.
• Uploads records, generates regulatory reports, and maintains patient charts.
• Coordinates with the interdisciplinary team to ensure holistic patient care.
• Acts as central point of contact for hospice team, patients, and families.
• Handles calls and messages, manages phone systems, and routes to staff.
• Supports pre- and post-IDT activities and prepares plans of care updates.
