Job summary

Compassus in Illinois seeks a Hospice Team Coordinator to model our pillars of Compassion, Integrity, Excellence, Teamwork, and Innovation. You will coordinate care, manage patient data, and ensure accurate documentation and billing alignment within HCHB and EMR systems.

You will serve as the central point of contact for the hospice team and families, supporting IDT activities, preparing plans of care, and ensuring clear communication across departments to deliver the highest level of patient

Qualifications

• High school diploma or GED required.
• Experience with medical records or medical office preferred.
• Strong communication and organizational skills.

Responsibilities

• Accurately enters and manages patient data, including referral intake and demographics.
• Ensures admission docs are complete, signed, dated, with CTI and order to admit; uploads to HCHB.
• Maintains patient records in EMR/HCHB and generates reports for regulatory compliance.
• Coordinates care among the IDT and serves as central contact for patients and families.
• Handles calls, messages, and phone system; prepares agendas and updates plans of care.
• Prepares binders for patients and assists with medical records requests.
