Job summary

Compassus is seeking a Hospice Team Coordinator to oversee delivery of hospice care, maintain comprehensive patient records, and support care coordination across the interdisciplinary team. You will ensure accurate admissions, CTI documentation, and timely communications with physicians, families, and facilities.

Responsibilities include data entry, document uploads to HCHB/EMR, and monitoring DME orders, while promoting Compassus values of compassion and integrity.

Qualifications

• Mathematical Skills: ability to add, subtract, multiply, and divide in all units of measure; compute rate, ratio, and percentages.

Responsibilities

• Accurately enters and manages patient data including referrals, admissions, and demographics.
• Uploads admission documents into HCHB and EMR; maintains accurate records and generates regulatory reports.
• Monitors DME and medical supplies orders; informs management of trends or issues.
• Coordinates with the interdisciplinary team (IDT) to ensure holistic patient care.
• Handles incoming calls, messages, and maintains a welcoming, responsive environment.
