Compassus is seeking a Hospice Team Coordinator to oversee delivery of hospice care and coordinate interdisciplinary teams. You will ensure data integrity, accurate charting, and timely communication with physicians, nurses, and families in a patient-centered environment.

The role emphasizes maintaining accurate records, coordinating IDT activities, and supporting the Hospice Director with pre/post-IDT tasks while upholding Compassus values and the six Pillars of Success.

Qualifications

• High school diploma or GED required.
• Two years of experience with medical records or medical office support preferred.

Responsibilities

• Enter and manage patient data and demographic changes.
• Ensure admission documents are complete and signed by the attending physician.
• Upload documents into EMR and HCHB systems for accurate histories.
