Job summary

Compassus is seeking a Hospice Team Coordinator to oversee the delivery of hospice care and maintain accurate records. You will coordinate with the interdisciplinary team, manage admission documents, and ensure smooth communication with patients and families.

The role emphasizes data accuracy, regulatory compliance, and efficient billing alignment while supporting the Hospice Director of Clinical Services and hospital communications.

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records, medical office, or business office support preferred.

Responsibilities

• Accurately enters and manages patient data (referral intake, new admissions, demographics).
• Ensures all admission docs are complete and uploaded to the HCHB system.
• Uses HCHB to maintain records and generate regulatory reports.
• Monitors DME orders and notifies management of trends or issues.
• Verifies and uploads paper visit records into EMR and HCHB.
• Ensures proper organization of documents in patient charts.
• Assists in ensuring billing aligns with level of care.
• Collects decertification paperwork and notifies discharge providers.
• Facilitates IDT coordination for holistic patient care.
• Acts as central contact for hospice team, patients, and families.
• Handles calls, messages, and phone system management.
• Supports DCS with IDT activities and UPOCs.
