Job summary

Bon Secours Home Care and Hospice by Compassus is seeking a Hospice Team Coordinator to model our Pillars of Success and coordinate delivery of hospice care. You will manage patient data, ensure complete admission documents, and maintain records across HCHB and EMR systems while coordinating among IDT members and families.

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records, medical office, or business office support preferred.

Responsibilities

• Accurately enters and manages various types of patient data, including referral intake information and updated demographics.
• Ensures all admission documentation is complete with CTI and order to admit.
• Uploads documents into the HCHB system for record-keeping.
• Uses HCHB to maintain patient records and generate reports for regulatory compliance.
• Monitors DME and medical supplies orders and informs management of trends.
• Monitors timely return of physician orders and processes them for efficient patient care.
• Verifies and uploads paper-based visit records into the EMR and HCHB for complete histories.
• Ensures organization of documents in patient charts for easy retrieval.
