Job summary

Compassus in Milwaukee seeks a Hospice Team Coordinator to oversee delivery of hospice care, coordinate patient records, and support the interdisciplinary team. You will ensure accurate data handling, timely documentation, and smooth communication with patients and families.

Qualifications

• High school diploma or GED required.
• At least two (2) years of experience with medical records, medical office, or business office support highly preferred.

Responsibilities

• Accurately enters and manages patient data (referral intake, newly admitted patient details, demographics).
• Ensures admission documents are complete, signed, dated, and include CTI and order to admit; upload into HCHB.
• Use HCHB to maintain patient records, generate reports, and add coordination notes as needed.
• Monitor durable medical equipment (DME) orders and notify management of trends or issues.
• Monitor timely return of physician orders and facilitate processing for care continuity.
• Verify and upload paper visit records into EMR and then into HCHB for complete histories.
• Organize uploaded documents in patient charts for easy retrieval.
• Ensure billed level of care matches the care level for each patient for accurate billing.
• Collect decertification paperwork and upload into patient records; notify providers on discharge.
• Initiate requests for medical records and hospital inpatient charts as needed; coordinate provider notifications.
• Facilitate coordination among the IDT for holistic patient care.
• Act as central contact for hospice team, patients, and families; ensure clear communication.
• Handle incoming calls, take messages, and route to appropriate personnel.
• Manage phone systems and switch to secondary system when needed.
• Assist DCS with pre/post-IDT activities and prepare/update plans of care (UPOCs); send UPOCs to attending physicians.
• Prepare patient binders for facilities; ensure essentials are organized.
• Perform other duties as assigned.
