AccentCare, Inc. is seeking a Hospice Care Consultant to drive hospice sales and build strong relationships with physicians, hospitals, and community partners in the Glendale, CA area. You will educate referral sources about our services and ensure timely, quality care for patients and families.

In this role, you will collaborate with Admissions and Marketing to craft outreach plans, coordinate with clinical staff, and uphold high standards of ethics and confidentiality while growing our program.

Formación

• A bachelor's degree and/or 5 years of proven experience in home health, healthcare sales, or hospice sales.
• Strong communication, problem-solving and relationship-building skills.
• Ability to work independently and manage a territory effectively.
• Knowledge of hospice regulations and services preferred.

Responsabilidades

• Develop and maintain trusting relationships with physicians, hospitals, skilled nursing facilities, and community organizations through in-person visits and educational conversations.
• Drive hospice sales by nurturing current relationships, identifying new referral opportunities and increasing awareness of our programs.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to develop and carry out community outreach plans aligned with our mission.
• Partner with clinical staff to ensure referrals transition smoothly into care with patient comfort prioritized.
• Participate in training and planning to uphold quality, ethics and confidentiality across activities.
