Aurora Health Care is seeking staff to participate in Admission, Discharge and Transfer processes under supervision by the hospice team. The role includes setting up rooms, orienting patients and families to the space and equipment, and maintaining patient dignity and comfort.

You will assist with vital signs, personal care, activities, toileting, and monitoring of catheters or tubes, while coordinating with nurses and physicians to support hospice patients.

Responsabilités

• Participates in the Admission/Discharge Transfer process under guidance of GIP Hospice Nurse or Clinical Supervisor
• Sets up patient’s room and orients patient and family to room including layout, call lights, bed functions, TV and telephone
• Obtains vital signs including height and weight
• Ensures patient belongings are accounted for and documented during admission/transfer/discharge
• Understands hospice philosophy and provides specialized care to terminally ill patients; provides post-mortem care
• Provides general patient care duties under supervision
• Provides personal care (bathing, oral care, shaving, backrub, skin care)
• Assists with activity orders, positioning, ambulation, ROM and transfers
• Assists with toileting, diaper usage and skin protection; responds to calls and patient requests
• Provides care for indwelling catheters or tubes; collects, prepares, and transports as directed
• Assists physicians with bedside procedures and oral suctioning as delegated
• Cooperates with other healthcare workers and supports patient nutrition needs
• Documents patient needs, problems and outcomes for care planning
• Demonstrates commitment to hospice team, coworkers and organizational goals
• Prepares documentation reflecting patient care; respects HIPAA/Privacy requirements; supports patient and family teaching; acts as patient/family advocate
• Maintains patient care standards and efficient unit operations
• Responds to emergencies and maintains patient safety including use of restraints and equipment; maintains ongoing CQI involvement
