Qualifications

• Must be able to read and write English proficiently.
• State tested Nursing Assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications.

Responsibilities

• Provide personal care and assist with ADLs according to the plan of care.
• Provide a safe environment for patients.
• Safely ambulate/transfer patient as appropriate.
• Assist with passive and active ROM exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under the supervision of the nurse, assist patient with pre-poured medications.
• Under the direction of the nurse, perform simple wound care and skin care as needed.
• Record in clinical record the activities provided and observations of patient.
• Complete charting, timesheets and mileage logs as defined by policy.
• Attend patient care conferences and team meetings.
• Demonstrate positive customer-service responsiveness according to agency standards.
• Maintain flexibility for variable shifts and weekends.
