Job summary

CommuniCare Family of Companies is hiring a Home Health Hospice Aide in Ohio. You will observe patients, document findings, and deliver personal care per plan of care, including bathing, feeding support, and helping with mobility.

Qualifications

• Must have STNA license in Ohio and active CPR certification.
• 18 years or older and able to work with minimal supervision.
• Schedule requires full-time availability and compliance with state/federal laws.

Responsibilities

• Observe patients and report/document observations and care performed.
• Provide personal care including bathing, dressing, and feeding support.
• Take vitals such as temperature, pulse, respiration, and blood pressure.
• Assist with ambulation, range of motion, and other care plan tasks.
• Support patients in self-administration of medications and communicate changes to RN.
