Job summary

Reserve Care, formerly Hospice of the Western Reserve, seeks caring STNA professionals to provide personal care under nurse supervision in home settings in Ohio. You will collaborate with a transdisciplinary team, assist with ADLs, and help with medications and basic care per the plan of care.

Qualifications

• Must be able to read and write English proficiently.
• Must be a State Tested Nursing Assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications where legally required.

Responsibilities

• Provide personal care under the supervision of a nurse according to the patient’s plan of care.
• Collaborate with transdisciplinary team in various settings.
• Assist with activities of daily living and prepare nutritional snacks.
• Assist with pre-poured medications under nurse supervision (Home Care only).
• Perform simple wound, skin, and bowel care as directed by the nurse.
• Document activities and observations in the clinical record.
• Attend care conferences and meetings as scheduled.
• Demonstrate positive customer-service responsiveness according to agency standards.
