Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking a Home Care Aide to provide personal care under nurse supervision per the patient plan. You will collaborate with a transdisciplinary team across settings to support patients and families.

Responsibilities include assisting with ADLs, maintaining a safe home, assisting with medications under supervision, wound and skin care as directed, and documenting visits and observations.

Qualifications

• Must be able to read and write English proficiently.
• Must be a state-tested nursing assistant in good standing.

Responsibilities

• Provide personal care and assist with ADLs according to the plan of care.
• Provide a safe environment for patients.
• Safely ambulate/transfer patients as appropriate.
• Assist with passive and active ROM exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Assist with pre-poured medications under nurse supervision (Home Care only).
• Perform simple wound/skin/bowel care protocols as directed by the plan of care.
• Record observations in the clinical record during visits.
• Complete charting, timesheets and mileage logs as required.
• Attend patient care conferences and team meetings.
